New Client Information Sheet

Thank you for choosing John Hughes Golf for your golf improvement needs! As a new client, I'd like to gather some information from you as well as provide you information about our cancellation policy. Please complete the entire form. When complete, you'll be directed to my Player Profile, where you can provide more detailed information about your golf game that I'll use to prepare for our first coaching session.
  • Credit Card Information

    Due to past experiences of new clients verbally securing their first golf coaching session and then failing to show up, we now require credit card information to hold/secure your reservation. Your card will not be charged prior to your arrival and yo will have the option of paying the associated coaching fees of your reservation by cash or credit card upon your arrival.
  • Cancellation Policy and Charge Authorization

    We ask that you provide at least 72-hours advance notice of any change or cancellation of your reservation to avoid any cancellation fees applied to your card. Should you fail to show within 20-minutes of your reservation time/date, a charge for the full amount of your reservation, as agreed upon when verbally making your reservation, will be charged to your card. By providing your credit card information and checking the box below, you agree to allow Florida Group Ventures CMM,Inc/d.b.a. John Hughes Golf the ability to charge your card for cancellation fees.
